Our full continuity of care allows us the ability to provide you with Medical Care at all levels and in multiple settings. We offer medical services in the following environments: in office visits, house calls, nursing facilities, hospice, and assisted living facilities. We understand hospital admission is sometimes necessary and because of this, we maintain close relationships with doctors in the local hospital settings. We can also provide assistance with admission to a local hospital. When you are discharged to your home, or a local skilled nursing facility, we will be able to serve your health care needs with seamless and efficient continuity. Our ability to provide continuity of care is what separates us from other medical practices in the area.
